This is the patient validation form in order to use it you must set the global flag in Administration > Globals > New Patient Form. Set the value to be 'Mandatory or specified fields only, use patient validation Laminas module'.

To set the modify form to use this patient validation set Administration > Globals > 'Modify Patient Form' to be 'Laminas Module check in addition to standard checks'.
